Dr. Anthony M. Pallotto
ProsthodontistDr. Pallotto is a board certified Prosthodontist. He received his Bachelor of Science degree in Biomedical Sciences from Marquette University in Milwaukee, Wisconsin. He then earned his Doctor of Dental Surgery from New York University and later his post-graduate training in prosthodontics from the University of Illinois, Chicago. Before joining ClearChoice Chicago, Dr. Pallotto treated patients at his private practice with a focus on complex implant related treatments utilizing all aspects of fixed and removable restorations. Through his research, work, and on-going training, he’s focused on bringing the best dental implant care to all of his patients facing diverse dental health challenges. He is a fellow of the American College of Prosthodontics and is a Diplomate of the American Board of Prosthodontics. Dr. Pallotto is passionate about his work and is excited each day at the opportunities to help his patients achieve high levels of health, function, and esthetics.

Dr. Neil Hagen
Oral SurgeonDr. Hagen graduated from the Dental School at Northwestern University in 1977. He served his residency at Northwestern Memorial Hospital, where he was Chief Resident for Oral and Maxillofacial Surgery from 1980-81. Dr. Hagen is licensed in Illinois for General Dentistry, Oral and Maxillofacial Surgery, Sedation and General Anesthesia, and Controlled Substances. Dr. Hagen has appointments at Northwestern Memorial Hospital, Children’s Memorial Hospital, and is an Assistant Professor at the Department of Surgery, Division of Dental Surgery, Northwestern University Medical School. Dr. Hagen is a member of the ADA, CDS, AAOMS, CSOMS and AO. He has vast experience, which has had him in high demand to lecture on Implant Dentistry since the early 1990s. He is a Diplomate of the American Board of Oral and Maxillofacial Surgeons and has certification in Advanced Cardiac Life Support.

Dr. Daniel I. Chin
Oral and Maxillofacial SurgeonDr. Chin received his dental education at the University of Michigan Dental School. He completed his surgical residency at a number of Chicago Hospitals: University of Illinois at Chicago Hospital, Michael –Reese Hospital, West Side Veterans Administration Medical Center, and Cook County Hospital. He is board certified by the American Board of oral and maxillofacial surgery, with hospital appointments at St. Alexius Medical Center, where he is also an assistant professor in the Department of Surgery. Dr. Chin is licensed in sedation and general anesthesia. Dr. Chin is also a Diplomate of the American Board of Oral and Maxillofacial Surgery, as well as a Fellow of the American Dental Society of Anesthesiology. He is also a member of the American College of Oral & Maxillofacial Surgery, the International Association of Oral and Maxillofacial Surgery, the Illinois Society of Oral & Maxillofacial Surgery, the Chicago Society of Oral and Maxillofacial Surgery, and the American Dental Association.

Dr. Phil Ruckman III
Oral and Maxillofacial SurgeonDr. Ruckman earned his Doctorate of Dental Surgery degree from the Indiana University School of Dentistry in 2010 and graduated with highest distinction. He received his specialty training in Oral Surgery at the University of Illinois Chicago. In addition to being a key member of the team at ClearChoice Dental Implant Center in Chicago, Dr. Ruckman has a keen interest in many areas of dentistry. He has been a member of the American Association of Oral Maxillofacial Surgeons and the American Dental Association. Dr. Ruckman believes in treating every patient exactly how he would like to be treated and loves being able to help others change their lives by giving them a new smile.

Anastasia Haupt
Oral and Maxillofacial SurgeonDr. Haupt earned her Doctorate of Dental Surgery at Creighton University School of Dentistry and graduated Magna Cum Laude. She completed her Oral and Maxillofacial Surgery certificate at the University of Illinois at Chicago. Dr. Haupt’s experience includes covering multiple hospitals and clinics, including the University of Illinois Hospital, Northwestern Memorial Hospital, and Jesse Brown VA Medical Center. Her area of knowledge includes outpatient sedation and anesthesia, corrective jaw surgery, as well as complex dentoalveolar and implant surgery. She is passionate about providing the outstanding care to her patients. Dr. Haupt is an Idaho native and loves the outdoors. In her free time, she likes to travel, run, and enjoy the diverse and delicious food throughout the Chicago area.
